Following, please completely fill out the Registration, Social Services & Consent
Form. Next, you will find our Privacy Notice, followed by an acknowledgement that
you have received and understand our Privacy Policies. Finally, the last form is the
Office Acknowledgements and Policies form. Please read carefully and sign
your name at the bottom of the letter.
Please make sure to bring all of these forms with you to your first office visit.
Do not mail them back to the office. Also, please remember to always
bring your picture ID, current insurance cards and your co-payment. If your
health insurance requires you to select a primary care doctor please do so prior to
your office visit. Please bring in any and all medication you take, in their
original bottles, to your appointment.
If the patient is under 18 years of age he or she must be accompanied by an
adult and will need to bring a copy of their current immunization certificate.

----NGDSonch;JmZfuW 2016 Year in Review Administrative OSEP Issues Policy Letter on Rights of Students with Disabilities Enrolled in Private Schools by Their Parents As you are most undoubtedly aware, the issue of students attending private schools is a hot right now. On December 27, 2016, the U.S. Education Department' Office for Special Education Programs (OSEP) issued a policy letter spelling out some of the key rights under the IDEA for students with disabilities when they are enrolled by their parents in private schools, as opposed to when they have been placed by the school district. Letter to Chambers, 117 LRP 2508 (Dec. 27, 2016, OSEP). The IDEA refers to these students as "parentally-placed private school students." These rules would apply any time a parent enrolls a student with a disability in a private school, including when using vouchers, tax credits and other school-choice options. Note, however, that an individual State may grant full IDEA rights to these students, and some States have done so. First, these students lose a number of their IDEA rights, including the rights to a FAPE, an IEP, and a due process hearing. Instead, school districts must meet with representatives of the parentally-placed private school students to determine which services will be provided to these students. Although the parents must be consulted in this process, the school district makes the final decision about which students will receive services, the services to be provided and where those services will be provided. OSEP went on to note that if transportation is necessary for a student to benefit from the services, the school district must provide transportation to and from the service site. This is so even if the site is outside of the school district's boundaries or even outside the State. School districts are not required to transport students between their home and the private school itself, however. OSEP went on to note that services may be provided on the site of the private school, even a private religious school, "to the extent consistent with law." In fact, OSEP stated that providing services on site may be preferred "so as not to unduly interrupt the child's educational experience, unless there is a compelling reason for these services to be provided off-site." If services are provided at another site, transportation may be required to get the student to and from the site. Finally, OSEP stated districts should coordinate with representatives of private school students to ensure services are provided at sites, including at the private school, so the district can avoid significant transportation costs. Department of Education Issues Dear Colleague Letter Regarding Education of Children with Disabilities Attending Public Virtual Schools U.S. Department of Education's August 11 release of an important and much needed "Dear Colleague" letter that will have a significant impact on children with disabilities and their families. The letter clarifies the obligation to ensure that students with disabilities attending public virtual schools receive all of the protections that they are entitled to under the Individuals with Disabilities Education Act (IDEA). This 15 year was the Manager of this Under 12 football team but that day, her elder brother who is the team's coach covered her duties as she had to be in school. Nonetheless, she resumed her duties as Manager after arriving at the field and assured her team that they could only do better and she was going to see to it. Within a few days, she gathered a group of friends who played football to help her brother with the coaching. The children were pleased with the closer attention during training and the huge dose of encouragement they were getting from these teenagers. It was more running and more drills but the children's enthusiasm never wavered. They believed their Manager and were determined to do better at their next game. After the training proper, the children and the teenagers would have a kick about. This was probably the most enjoyable part of the day for all and their happiness was noticed by a passer-by who came up to talk to us. He told us that he was amazed to see boys and girls, teens and even adults playing together so harmoniously. He then asked our permission to take a few pictures and some footage with his camera. A couple of days later, he sent us a clip of what he took and we learnt that he was Clifton See from Incubate Productions. He entitled the clip "Whampoa All Stars" and he told us to let all the players know that they were all stars regardless of whether they won a game or not. We could not agree with him more and will be showing the clip and relaying the message to all the children before their next game. Education (Ed.D.) in Educational Leadership, designed for current higher education professionals
who aspire to leadership positions on college or university campuses. We look forward to receiving

As you complete your application, keep in mind the following admission criteria:
1. Master's degree from an accredited institution of higher education in a discipline or
professional field that is relevant to the Ed.D. in Educational Leadership.
2. A 3.00 or higher cumulative average (GPA) in all graduate coursework.
3. Two or more letters of reference from leaders in postsecondary education familiar with
your work. Ask your references to use the form on the next page.
4. Résumé that illustrates important work-related experiences with an emphasis on yo ur
work as a leader at postsecondary institutions of higher education.
5. Acceptable scores on the General Test of the Graduate Record Examination (GRE) taken
within five years of your application.
6. A personal statement covering six important topics:

• Intended area of individual specialization

And remember that setting up is part of the team's work, not something your parents or your coach should do for you… For the judging rounds on Saturday and Sunday you will get a timeslot.
