Enclosed are the medical clearance documents and consent forms the athletic department requires you to have completed prior to participation inany intercollegiate athletic activity. Please take time to review the forms thoroughly and complete all ofthe requested information. All of the enclosed forms must be completed and turned in to the Athletic Training Department prior to you participating in any intercollegiate practice or completion. Athlete Physical Screening Exam All students participating in intercollegiate athletics at Merced College are required to have an athletic physical completed every year by an MD, DO, NP, or PA. No other healthcare provider may clear a student for athletic participation. Merced College will hold a physical screening on campus for Fall athletes in July/August and a second screening will be held for Spring athletes in January. For exact dates and times please consult with your coach. If you are unable to attend the on campus screening you will be responsible for having your physical screening exam completed by your own physician. Completing a physical does not ensure clearance for participation. The Athletic Training Department must review and approve the physical packet before the student-athlete may participate. Health Insurance Verification All student-athletes with medical insurance must provide current information for intercollegiate participation. The student-athlete will need to complete the Health Insurance Record form and provide a front and back copy of their insurance card. Student-athletes who do not have insurance will be covered under Merced College's Student Health insurance policy. This student policy becomes a secondary policy to your primary insurance coverage. Further information about the student insurance policy can be provided upon request. Health History The medical history form must be filled out in its entirety and as accurately as possible. Any falsification or omission of injuries or severity of health conditions is grounds to have the student-athlete's medical eligibility revoked. Please make sure to bring all of these forms with you to your first office visit.

We are very pleased that you are interested in the Higher Education Strand of CCSU's Doctor of
Education (Ed.D.) in Educational Leadership, designed for current higher education professionals
who aspire to leadership positions on college or university campuses. We look forward to receiving

As you complete your application, keep in mind the following admission criteria:
1. Master's degree from an accredited institution of higher education in a discipline or
professional field that is relevant to the Ed.D. in Educational Leadership.
2. A 3.00 or higher cumulative average (GPA) in all graduate coursework.
3. Two or more letters of reference from leaders in postsecondary education familiar with

4. Résumé that illustrates important work-related experiences with an emphasis on yo ur

Burghausen's castle, with its length of more than a kilometer, presents the biggest castle complexe in Germany. The outdoor areas are open to the public all year round. We will have two hours in the outdoor areas and the city of Burghausen. After that we will move on to Prien am Chiemsee. Located on an island in the middle of the lake, castle Herrenchiemsee attracts thousands of visitors each year. We will take the ferry to visit the castle, built by king Ludwig II, based on the model of Versailles.. We want to spend an unforgettable weekend with you in the Alps. We will go on a hiking trip near the mountain resort GarmischPartenkirchen where we will spend 2 nights in the Kohlstattalm Hut and hike to the Benediktenwand mountain ridge. The itinerary will include hikes through a mountainous panorama on all three days as well as evening entertainment in the hut. Important details on the mountain hut: We'll be sleeping in a large dorm. There is a kitchen for communal cooking. Toilets and basic washing facilities are also at the hut. Located directly at the entrance of the picturesque Danube Gorge, the 600- year old Weltenburg is held to be the oldest monastery in Bavaria. There we will have a sausage breakfast in one of the most beautiful beer gardens in Bavaria. Afterwards we will take the ferry through the Danube Gorge to Kelheim from where a small hiking trail leads to the Liberation Hall of Ludwig I. The building finished in 1863 is not only impressive on its own, but also offers a breathtaking view over the Danube valley.. It contains details about the program of the city's theater and other smaller stages, announcements of concerts, events of the adult education center, exhibitions and many more. Cheap Theater Tickets Students always get a 30 percent student discount on tickets for the city theater. There is also the possibility of so-called „Last-Minute-Tickets", which cost only 7,50 Euro if there are still tickets left 15 minutes before the start of a show.
