The first sheet will help us learn more about you; please completely fill out this
form about your family history. The next sheet is titled, "Authorization for the
use and/or Disclosure of Protected Health Information", and you will need to
completely fill that out for our doctors to treat you to the best of their ability; it
gives us permission to review your medical records from your previous primary
medical facilities.
Following, please completely fill out the Registration, Social Services & Consent
Form. Next, you will find our Privacy Notice, followed by an acknowledgement that
you have received and understand our Privacy Policies. Finally, the last form is the
Office Acknowledgements and Policies form. Please read carefully and sign
your name at the bottom of the letter.
Please make sure to bring all of these forms with you to your first office visit.
Do not mail them back to the office. Also, please remember to always
bring your picture ID, current insurance cards and your co-payment. If your
health insurance requires you to select a primary care doctor please do so prior to
your office visit. Please bring in any and all medication you take, in their
original bottles, to your appointment.
If the patient is under 18 years of age he or she must be accompanied by an
adult and will need to bring a copy of their current immunization certificate.
Please arrive 15 minutes ahead of your scheduled appointment time so that if
you have questions about these forms or we need more information, we can

Dear Prospective Ed.D., Higher Education Strand Applicant:
We are very pleased that you are interested in the Higher Education Strand of CCSU's Doctor of
Education (Ed.D.) in Educational Leadership, designed for current higher education professionals
who aspire to leadership positions on college or university campuses. We look forward to receiving
your application.
As you complete your application, keep in mind the following admission criteria:
1. Master's degree from an accredited institution of higher education in a discipline or
professional field that is relevant to the Ed.D. in Educational Leadership.
2. A 3.00 or higher cumulative average (GPA) in all graduate coursework.
3. Two or more letters of reference from leaders in postsecondary education familiar with
your work. Ask your references to use the form on the next page.
4. Résumé that illustrates important work-related experiences with an emphasis on yo ur
work as a leader at postsecondary institutions of higher education.
5. Acceptable scores on the General Test of the Graduate Record Examination (GRE) taken
within five years of your application.
6. A personal statement covering six important topics:

• Intended area of individual specialization
• Reasons for pursuing a doctorate

September 17, 2020 – Campuswide email
Dear Students, Faculty, and Staff,
We are now a few days past the Block 1 midterm, which means we are beginning the home
stretch for the first half of the admittedly wacky, COVID-style fall term at Centre College. While
it may feel like we have the wind at our backs, we can't get lackadaisical or be over confident.
Every day requires us to be focused and vigilant to stay our course.
Having delivered meals to students in isolation and quarantine, I know firsthand from my
conversations with them that this situation is in no way desirable. Yes, everyone makes their way
through it, but we must continue to do everything we can to keep our test result numbers as low
as possible. To this end, remember to always practice the basics: mask, distance, and wash
hands. For those of us on campus this fall, please limit your travel off campus to emergencies.
And on the test resu lts, our overall positivity rate in our first four weeks on campus is 0.36
percent, which is really quite good. While just a few results are still pending from this week's
student surveillance testing, we have no new positive results to report as I write to you this
afternoon. Again, stay the course, check the dashboard for regular updates, and protect your
Centre home.
As an expression of gratitude to our students in particular, we are working on some swag, the
first of which has been ordered and should be delivered in about two weeks. Thanks to all 800+
students who voted for their favorite designs in a survey sent out earlier this week by our
Communications office. Prizes selected through a random drawing for those who participated
should be announced on Friday.
On a related note, I am pulling together a team of students and staff to make sure we are doing
all we can to organize fun and enjoyable activities on c ampus outside of the classroom and labs,
and beyond the practice fields, to unwind and relax in ways that are safe and healthy. More on
this effort soon.
A few other items:
• Calendar conversations for both CentreTerm and the spring term began in earnest this
week. We are currently discussing the calendar revisions with faculty and student leaders
for their viewpoints. As with all things at this moment in time, we will need to be flexible.
Of course, the possibility and timing of a vaccine is a critical factor in considering things
such as breaks. Without a widely accessible vaccine, it is likely we will need to continue
to maintain the same health and safety protocols on campus for at least part of the
spring. Look for news about CT and the spring calendar in the coming weeks.
• A recent audit of study space usage suggested, at least in the case of Hazelrigg, that this
is currently underutilized, likely due in part to the wonderful weather we are having in
Kentucky. We want students to have helpful places to study while the library is closed, so
we will continue to have a variety of indoor spaces available on campus as the weather
turns cooler in the coming weeks. For now, I hope you are taking advantage of the great
weather and studying outside whenever possible.
• While I believe that the significant decrease of off-campus travel is one of the key factors
keeping our positivity rates low, I do appreciate that exceptions to the rule may exist. A
new campus leave request form has been added to the Social Contract and is available
HERE. Please use this form whenever you go off campus for more than a quick trip to
pick up an order at a local business. As noted on the form, students should avoid
traveling off campus, as part of the Social Contract, except for emergencies or approved
events (such as a funeral or doctor appointment).
• As part of our effort to keep lines of communication open, more Zoom forums are being
scheduled, including one for staff next Monday afternoon, another for parents and
families on Tuesday evening, and a third for our Parents Council on Saturday morning.
Finally, work is underway to plan for our fall meeting of the Centre College Board of Trustees,
which will again be virtual. Our trustees have been incredibly supportive of our work to contain
the coronavirus on campus. This will be my first official meeting as president, and I look forward
to providing them with updates on what we have been doing as well as begin to outline a vision
for our future through the lens of 3-, 5-, and 10-year projections. As I shared at the Faculty and
Staff Opening Conference, last year's Third Century Conversations have provided an impressive
set of insights that will help drive us forward as we enter into a full-blown strategic planning
process in the near future.
With little precipitation in the forecast for the coming week, though cooler temperatures seem
to be on their way, I encourage as much outdoor activity as possible, albeit while safely masked
and socially distanced. Our dog, Blue, has been a great ambassador to help Dina and me meet
new people when we are out on walks, and we look forward to expanding our circle of friends in
the Centre family this weekend.
